Please have your healthcare provider fill out this prescription for a "breast pump" at your next appointment and ask them to fax it to 844-276-5457. Please note that while we encourage you to order your pump early, most insurance plans have contractual requirements for when we can dispense the pump. Our office staff will notify you when your pump is available for shipment. You will receive a confirmation email with a tracking number when your pump ships. Submitting a pump order online through HBHM is not a guarantee of coverage. Please get in touch with your insurance provider for full coverage information. Eligible pump models may vary based on your insurance coverage, which is subject to verification, and may require an upgrade fee.
