To request your Medela BabyWeigh™ Scale rental, please fill out the form below. Our office will contact you to go over our rental contract and process the first month’s payment of $100 / month. We cannot ship a scale until we have received a completed rental contract and a full month rental is paid upfront. We will bill you monthly after that using the information you provide on our rental contract. Please print out this contract and sign both sides of the document.
