Please have your healthcare provider fill out this prescription for a "breast pump" at your next appointment and ask them to fax it to 401-884-5541. Please note that while we encourage you to order your pump early, most insurance plans have contractual requirements for when we can dispense the pump. Our office staff will notify when your pump is available to be picked up or if you choose to have it shipped, you will receive a confirmation email with a tracking number when your pump ships.
